Made in Sepulveda, California by Thomas Organ Co. for Vox. in the early to mid 1970s. Pedal can be used as a wah, fuzz, or volume pedal. 

Pedal is in poor condition. All functions on the pedal engage and work, but the pots are  scratchy and there is quite a bit of noise when engaging the fuzz circuit.  The fuzz circuit doesn't seem as loud as it should be. There is a small crack on one of the corners of the bread-board and it is not screwed down in chassis.

Chassis has quite a few scratches and light rusting, but is in decent condition considering its age. Fuzz bypass switch is in good condition. Everything on it is original.

This would be a great project for someone mechanically inclined. It likely just needs a deep cleaning and few new parts to revive everything.
